It is the policy of the company that the Governing Body documents the appointment of professional staff members.  Only properly documented, licensed, and trained professional staff members are permitted to provide patient care services.

Investigation of Claims, Licensure, Education, and Experience

It is the policy of the company to investigate claims made by potential Providers in the areas of education, certification, licensure, and work related experience.  In order to validate a candidate’s education, the company contacts the college or university attended to verify.  In order to validate a candidate’s certification and licensure, the company uses the appropriate State website to verify possession of a current license in good standing or verifies certification through their credentialing body if in a non-licensure state.  In order to validate a candidate’s work related experience, the company contacts the candidates former direct supervisor to validate employment dates and experience.

Proof of Licensure, Education, and Continuing Education

A copy of the provider’s current license is kept in the Personnel File to ensure all employed providers have a license in good standing.  A copy of the providers diploma is maintained in their Personnel Files to ensure a proper institution has properly trained all providers.  All continuing education statements and proof of attendance are also maintained in the provider’s Personnel File, to ensure that all Providers have taken the necessary courses in the required time period consistent with the services provided to patients.

Annual Review

All documents within a provider’s Personnel File are reviewed once a year during the Provider’s annual review.  This review also includes a validation of current license in good standing by utilizing the Professional Regulation section of the State’s website to check the status of the provider’s licensure and that the provider does not appear on the OIG exclusion list.
